New Students
In order to register your child, please complete the Kahnawake Survival School Registration Form.
Paper copies of all forms are available at the Kahnawà:ke Education Center Main Office. Copy/printing of required documents is available if necessary. All applications, online or paper copies must be submitted with ALL of the required documents in PDF.

All online forms are fillable forms.
To use a fillable form:
- Click on the fillable form you would like to use.
- Fill in the form completely.
- Save the filled-in form on your device as your child's name.
- For elementary or high school send the saved file to Tracey Alfred.
Please note:
*Students not currently attending either Karonhianónhnha Tsi Ionterihwaienstáhkhwa or Kateri School must submit the most recent report card and if applicable, IEP and/or reports and assessments.
*Paper copies for all current Grade 6 Students from Karonhianónhnha Tsi Ionterihwaienstáhkhwa or Kateri School will receive paper copies in the mail.
Current or Returning Student Forms
Current students are sent re-registrations forms during the registration period which are to be returned to the students' respective school.
New application forms are NOT REQUIRED.
